If you require any vaccinations relating to foreign travel you need to complete a Travel Risk Assessment form. This can be completed online or a hard copy can be collected from our Reception Desk. This will include questions on which countries and areas within countries that you are visiting to determine what vaccinations are required. When completed the form can be assessed by our Travel Nurse to determine which vaccinations are required.

It is important that once your form has been completed and assessed by our Travel Nurse to make an appointment as early as possible to receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Please allow one week for your completed questionnaire to be assessed by our Travel Nurse before contacting Reception who will advise you if an appointment for Vaccinations is required and will book an appropriate appointment for you.
